There are times that you move from one beauty school to another in the same state or between the state. The school will need your certification of completed hours as registered in the KY State Board of Cosmetology before your continuing education can be enrolled in the school. To do that, the school will have to fill out the application below to request that certification of hours/license on the student’s behalf.
The student will need to fill out this online agreement and give the school (Louisville Beauty Academy) permission to request that information on your behalf.
