Before You Enroll: Written Document Checklist
Serious education starts with written clarity. Louisville Beauty Academy welcomes questions, walk-ins, tours, family discussion, translation support, and affordability review. Before any student relies on a price, schedule, discount, payment term, transfer statement, licensing expectation, or enrollment promise, the student should ask for the current written document that controls that issue.
This checklist is public student guidance. It is not legal, financial, tax, immigration, medical, or licensing advice. Current signed enrollment documents, current school policy, Kentucky Board of Cosmetology requirements, and applicable law control.
Start Here: Student Handbook / Official Student Catalog
The Student Handbook / Official Student Catalog should be reviewed before or alongside any enrollment decision. It should be read with the current enrollment agreement, program disclosures, refund policy, attendance and progress rules, grievance process, written request forms, and any current written document issued for the student’s specific program and enrollment date.
1. Program and Cost Documents

- Current program name and required hours.
- Current standard contracted cost.
- Current books, kit, supplies, fees, deposits, and payment timing.
- Any possible incentive, affordability review, price-match review, or scholarship-like reduction in writing.
- Clear statement that reductions are conditional unless written into the student’s enrollment terms.
2. Schedule, Attendance, and Completion Documents
- Expected schedule or available schedule options.
- Attendance, clock-hour, absence, leave, re-entry, and make-up expectations.
- Timekeeping rules and any consequences for inaccurate or dishonest time records.
- Graduation workflow: hour completion, Board submission, confirmation, and certificate timing.
3. Licensing and Board-Controlled Items
- Current Kentucky Board of Cosmetology requirements for the program.
- Current examination, permit, license, transfer, or age-related rules when applicable.
- Written understanding that LBA cannot guarantee Board approval, exam result, license timing, transfer acceptance, employment, income, or business outcome.
4. Student Responsibility Documents
- Student catalog, handbook, or applicable student policy.
- Professional conduct, sanitation, safety, zero-disruption, and grievance procedures.
- Written communication path for questions, concerns, complaints, records, and payment issues.
- Privacy and adult-student communication expectations, including parent or third-party access limits.
5. Practical Rule
If an issue matters, ask for it in writing. If a statement is only verbal, old, unclear, copied from a third party, translated informally, or found in an older webpage, ask LBA to confirm the current written version before relying on it.
Easy next step
Ask LBA to help you choose the right path.
If you are comparing programs, cost, schedule, language support, tour options, or enrollment documents, use this form or text enrollment. LBA can follow up with current written information so the next step is clear.
Important: Form submission or text follow-up does not enroll a student, guarantee a discount, guarantee licensure, or create a contract. Current written school documents, signed agreements, official Kentucky Board of Cosmetology requirements, PSI rules, and applicable law control.
